You have the right to choose your site of care!
Switching your ongoing infusion or injection treatment to Revive Infusion is easy. As a first step, fill out the New Patient Inquiry form online, and a member of our team will be in touch with next steps.
We will also need a physician’s order for your medication, details about your health insurance plan, and other information such as your demographics, vitals, labs, and certain clinical data to authorize your treatment. All of the information your doctor needs to provide can be found on our Referrals page.
Once we have everything from your doctor, Revive Infusion will reach out to your insurance plan to obtain a prior authorization (PA). Simply put, a PA is an approval from your insurance plan that the prescription is covered by your plan and its cost will be reimbursed.
(NOTE: Revive Infusion requires a prior authorization before the administration of any therapy.)Once we receive the prior authorization, we can order your medications and schedule your infusion for a time that is convenient for you.

Yes. Revive Infusion can utilize a patient’s existing IV port to administer its therapies.
(NOTE: Revive Infusion does not place new IV ports. This procedure must be completed by a physician prior to your appointment.)An IV port is a device that is implanted under a patient’s skin for the administration of IV therapies. Ports can simplify the IV process by eliminating the need for a new IV placement and stick for every infusion. IV ports are often used when a patient will be receiving IV therapy for an extended period of time.

Revive Infusion follows all of the same regulatory guidelines and safety standards as your physician’s office and most hospitals. Revive Infusion staffs its centers at a low patient-to-nurse ratio, ensuring a level of oversight and rapid response necessary if you experience any adverse event from your treatment. Our physician supervisors are available to us should a problem arise with your infusion, however our first step is to call your prescribing doctor.
In addition, each of our centers is equipped with automated external defibrillators (AEDs), and our team is trained in administering basic life support and anaphylaxis protocol. Revive Infusion takes every measure to maintain a strict aseptic technique, including during medication preparation and administration.
